PROGRAM SERVICES

The Home & Community Care Program works in partnership with other service providers and client and families to increase the effectiveness of community care and to eliminate gaps in and duplication of services. Our services are managed effectively and efficiently, demonstrating accountability for the use of health care funds.

The Home & Community Care Program provides services such as client assessment with development of goals and objectives. Our program provides, wound care, foot care and other nursing procedures such as health education and teaching, medication management and administration & monitoring of diabetes and other health issues. The program staff also provides case management services such as discharge planning, consultation with physicians and others, and charting each interaction with the client as they proceed towards meeting their goals. Personal care aides provide foot care, personal care such as bathing and assistance with hygiene routines, transporting and working with clients at luncheons and feasts.

The care aides provide support and encouragement on their home visits to clients. Personal care aides also provide a small amount of assisted living help, but mostly provided by the homemakers. The Home Care nurse does Homemaking assessments on clients to identify home maintenance needs. This is provided to the Social Development Worker for action.

A library of educational resources for is available on conditions such as diabetes management, arthritis, asthma/COPD, palliative care, nutritional and exercise, lifestyle issues, and oral health.

ACTIVITIES

  • Maintenance of information, ongoing planning, record keeping, reporting and evaluating 

OBJECTIVES

  • Plan, develop and deliver comprehensive, culturally sensitive, accessible and effective home care services.
  • Assist First Nations with chronic and acute illness in maintaining optimum health, well being and independence.
  • Ensure that the people of the communities of Coldwater, Shackan and Nooaitch have access to a continuum of services within the community.
  • Facilitate the effective use of home care resources through a structured, culturally defined and sensitive assessment process to determine service needs of clients and the development of a care plan.
  • Assist clients and their families in participating in the development and implementation of the client’s care plan to the fullest extent and to utilize available community support services where available and appropriate in the care of clients.
  • Deliver home care services through continuing education sessions, evolving technology, and the development and implementation of information systems that enable program monitoring, research, defining best practice and evaluation. 

GOALS

  • Assist people to live in the community as independently as possible, preserving and encouraging enhancement of the support provided by the family and community
  • Work in partnership with families and other service providers to increase the effectiveness of community care and to eliminate gaps in and duplications of services-manage services effectively and efficiently, demonstrating accountability.
