CHN PROGRAM SERVICES

The community health program includes communicable disease control, pre/post natal health, school health, and immunizations.  The community health role also includes administrative duties so a great deal of time is spent on the phone, doing paper work, and assisting with orientation new staff.

CHN ACTIVITIES

  • Family Planning

  • Nutrition Awareness
  • Breastfeeding
  • Maintain vaccination status
  • Immediate reporting regarding CD occurrence and necessary follow-up including contact tracing and treatment
  • Immunizations: baby, child, school and community
  • Prenatal clinics/classes & Pre/Post Care
  • Promote health care through counselling, workshops, community and school awareness campaigns including HIV/STDs, hearing and eyesight screening, nutrition, dental care, etc.
  • Offer preventative education and health promotion themes including diabetes, arthritis, food safety, heart smart, Alzheimer disease, etc.
  • Ensure safety, maintenance and correct storage of equipment and supplies (including vaccines, biologicals, needles and syringes

CHN OBJECTIVES

  • To administer a Community Health Nursing Program that enhances the health and well-being of the First Nations people living within the Coldwater, Shackan and Nooaitch communities.
  • The CHN will plan, provide and evaluate a primary community health service to the communities of Nooaitch, Shackan and Coldwater Bands.  Services will provide a multi-disciplinary team process in carrying out health care and advisory service to the Society and SCHSS communities.
  • Decrease the incidence of detrimental effects of communicable diseases
  • Attain and maintain optimal mental, physical, spiritual and emotional health
  • Ensure that individuals have access to health education and intervention
  • Ensure efficiency and effectiveness of service through professional development and sound management

CHN GOALS

  • To decrease the incidence of, and control outbreaks of, communicable disease in the community through immunization, treatment, and education.
  • To maximize and maintain the optimal physical, mental, spiritual, and emotional health of individuals, families and communities while remaining sensitive to the cultural needs of those individuals, families and communities.
  • To ensure efficiency & effectiveness of the CHN service within all three communities through professional development & sound management
  • To improve maternal and infant health both prenatally and postnatal

CHA PROGRAM SERVICES

Through education, research and advocacy, members work together to create an environment for Aboriginal communities to achieve the vision of holistic well being.  With SCHSS CHA’s share the task of maintaining high standards through monitoring water quality, community health duties, infant massage and creation of health and wellness programming. 

CHA ACTIVITIES

  • Assist the CHN in baby clinics and vaccinations throughout the year
  • Conduct home visits, hospital visits and follow-up to focus on personal health, promote prevention of illness and accidents, and to encourage healthy and independent lifestyles
  • Collect and process water samples using the Colilert System on a regular basis at designated sites in addition to collecting, preparing other samples for shipping to the Vancouver Lab

Client advocacy for community members (an ongoing process through policy development and changes to programs)
  • Provide home and health safety checks on a regular basis or when requested by a community member 

CHA OBJECTIVES

  • Facilitate and promote community involvement in planning and the delivery of community health services
  • Administer and provide community health services that enhance the health and well-being of First Nations people living in the Coldwater, Shackan and Nooaitch communities
  • Facilitate promotion, prevention, education, policy/administration translation, and follow-up on health issues
  • Promote healthy life choices using the SCHSS health framework
  • Provide a focus for health promotion and prevention activities
  • Professional development for in-service staff
  • Offer a broad range of community-based activities that enhance the health and well-being of the people living in participating communities
  • Ensure that individuals have access to health education and intervention
  • Ensure efficiency and effectiveness of service through professional development

CHA GOALS

To deliver high levels of medical education and programs for the betterment of the community member while maintaining quality standards of one’s physical, spiritual, mental and emotional needs.
